A company is looking for a Triage Registered Nurse - Home Health Remote PT Weekend Variable. Key Responsibilities Collect and analyze data from telephonic contacts and EMR to resolve patient issues Develop and implement care plans while coordinating care delivery and evaluating outcomes Delegate care elements to appropriate healthcare workers and promote shared governance Required Qualifications, Training, and Education Graduate from an accredited School of Nursing; BSN preferred Current RN license or temporary license in the state of practice Basic Life Support for Healthcare Provider certification required Additional education or training may be required by the nurse leader Ability to meet physical requirements of the role, including lifting and responding to patient conditions
