Summary
The Palliative Care Registered Nurse is a professional Registered Nurse (RN) who provides skilled clinical care, demonstrates advanced assessment, coordination with interdisciplinary teams, and advocates for patient dignity and end-of life care. He / she is accountable for adhering to established policies, standards of care and standards of practice.

The Palliative RN assumes the following major duties and responsibilities :
1. Collects comprehensive data pertinent to the patient's health and / or the situation.
2. Analyzes the assessment data to determine the patient's diagnosis or issues.
3. Identifies in partnership with the interdisciplinary health care team, expected outcomes for a plan individualizes to the patient or situation.
4. Develop a plan that defines strategies and alternatives to attain expected outcomes.
5. Advocate at the CLC / Palliative Care Interdisciplinary Care Team meetings, ensuring the continuum of care is addressed and readdressed based on the patient's need.
6. Implements the identified plan.
7. Evaluates progress towards attainment of outcomes.
8. Responsible for physical care, including the patient's physical condition and comfort, yet must also possess some level of expertise in psychosocial and spiritual aspects of care, to address the patient and family from a holistic perspective.
9. Documents using the CARIBOU software system.
10. Educates the patient and family regarding physician care, which includes such things as medication administration, equipment uses, skin care, nutrition, catheter care and transfers.
11. Participating in continuous unit performance improvement, performance, and quality measures as well as CLC compare and CLC Sail data.
12. Knowledge of the Long-Term Care Survey, CARF and Joint Commission requirements and standards.
13. Identifies and addresses learning opportunities with educational in-services and performance improvement plans.
14. Performs other tasks / duties as assigned.
Preferred Experience : One year of experience in LTC / SNF / Rehabilitation or hospice care is preferred.

Demonstrates proficiency in practice based on conscious and deliberate planning. Self-directed in goal setting for managing complex client situations.
