Job Description

Seeking a Full-time or Part-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing and Assisted Living Facilities in Keene, NH, and nearby areas.

• Conduct patient exams, review medical records, and order diagnostic tests.
• Diagnose and treat health conditions, prescribe medications, and develop treatment plans.
• Perform wellness visits, medication reconciliation, and educate patients and families on health management.
• Maintain accurate records, promote health practices, and oversee overall patient health.
