SUMMARY : The Nurse Practitioner is a licensed practitioner who is responsible for managing health problems and coordinating health care of the sub-acute and nursing facility patient in accordance with state and federal rules and regulations and the nursing standards of care. Appropriate clinical management is conducted in collaboration with other healthcare team members and physicians.

EDUCATION, QUALIFICATIONS, CREDENTIALS :

• Graduate of an accredited Nurse Practitioner Program
• Current and / or pending Professional Licensure
• Current Nurse Practitioner certification in the specialty area
• Current provider card in Basic Life Support (BLS)
• Minimum of one (1) year of experience as a Nurse Practitioner; experience in sub-acute care and / or geriatrics preferred, but not required
