Your mission : provide exceptional, evidence-based therapy tailored to each patient's needs, collaborate with interdisciplinary team members to develop and execute individualized care plans, and empower patients to achieve their recovery goals. Key Responsibilities

Patient Care Excellence

  • Individualized Care : Deliver high-quality, personalized therapy interventions that improve patients' mobility, independence, and overall quality of life in their home environment. Ensure patient goals and well-being remain the central focus of all treatment plans.
  • Evidence-Based Practices : Stay current with advancements in physical therapy and integrate evidence-based techniques to enhance patient outcomes and care delivery.

Operational Excellence

  • Therapy Plan Development : Evaluate patients' conditions and create tailored treatment plans that align with their goals and health status, adjusting interventions as necessary to ensure optimal results.
  • Performance Metrics : Monitor patient outcomes, track therapy progress, and document results to maintain clinical excellence and compliance with organizational and regulatory standards.
  • Team Collaboration : Collaborate with interdisciplinary team members, including nurses, social workers, and other therapists, to provide holistic care that supports overall patient outcomes.
  • Productivity and Efficiency

  • Caseload Management : Effectively manage a diverse caseload, ensuring timely visits and adherence to care schedules while maintaining a high standard of care.
  • Accurate Documentation : Complete thorough and timely documentation in the electronic health record (EHR) system to support care continuity, compliance, and billing accuracy.

  • A Typical Day

  • Morning Patient Review : Begin the day by reviewing the schedule and patient care plans, ensuring all necessary equipment and resources are ready. Prepare for the day's visits by familiarizing yourself with patient histories, goals, and therapy needs.
  • Patient Home Visits : Conduct home visits to evaluate patients' physical conditions, provide therapy interventions, and educate patients and families on exercises and techniques to enhance recovery. Tailor each session to the patient's unique needs and adjust treatment plans as necessary.
  • Team Collaboration : Mid-day, participate in interdisciplinary care team discussions or meetings to review patient progress, share insights, and collaborate on care plans. Engage with nurses, social workers, and other therapists to ensure coordinated and holistic care.
  • Afternoon Documentation : Complete detailed and timely documentation of patient visits, therapy interventions, and progress notes in the electronic health record (EHR) system. Ensure documentation is compliant with Medicare and organizational standards to support billing and continuity of care.
  • Patient Follow-Up and Education : Follow up with patients or families via phone or telehealth to answer questions, reinforce exercise plans, and provide additional support as needed. Address any concerns to ensure satisfaction and build trust.
  • Evening Reflection and Planning : Conclude the day by reviewing patient outcomes and therapy progress, setting goals and priorities for the following day. Reflect on interventions to identify opportunities for improvement and ensure continued patient success.
  • Your Expertise

  • Education : Bachelor's or Doctorate degree in Physical Therapy (DPT) from an accredited program. Must hold an active state licensure as a Physical Therapist.
  • Experience : A minimum of 2 years of experience in physical therapy, preferably in a home health or similar patient- centered care setting.
  • Patient-Centered Care : Proven ability to develop and implement personalized treatment plans that improve mobility, independence, and quality of life.
  • Clinical Expertise : Strong knowledge of evidence-based practices, therapeutic techniques, and compliance with home health regulations.
  • Communication Skills : Exceptional interpersonal and communication skills to build trust with patients and collaborate effectively with interdisciplinary teams.
  • Problem-Solving : Ability to adapt care plans to meet the evolving needs of patients while managing complex medical conditions.
  • Time Management : Skilled at managing a dynamic schedule, balancing patient care needs, documentation, and team collaboration efficiently.
  • Tech-Savvy : Proficiency in using electronic health record (EHR) systems and telehealth tools to document care and engage patients effectively.
  • Regulatory Knowledge : Familiarity with Medicare and Medicaid guidelines as well as state and federal home health regulations.
  • Your Work Environment

  • Location : Primarily in patients' homes with occasional travel to the office for team meetings or training. Regular travel within the assigned region is required to provide care.

  • Productivity : Meeting or exceeding expected caseload and visit productivity standards while maintaining high-quality care and timely completion of patient visits. Staying on schedule and ensuring punctuality for all patient appointments.
  • Key Performance Indicators (KPIs) : Achieving measurable goals related to patient outcomes, therapy progress, and adherence to care schedules. Regularly tracking KPIs such as visit completion rates, documentation timeliness, and patient functional improvements.
  • Clinical Outcomes : Ensuring measurable improvements in patient mobility, strength, and independence through effective therapy interventions. Reducing hospital readmissions and achieving positive long-term patient health outcomes.
  • Patient Satisfaction Scores : Delivering compassionate, patient-centered care that results in high satisfaction scores from patients and their families. Actively addressing concerns and building trust to foster strong relationships and confidence in the care provided.
  • Team Collaboration : Demonstrating strong teamwork by actively participating in interdisciplinary meetings, sharing insights, and contributing to care planning. Supporting colleagues and fostering a collaborative environment to achieve shared goals.
  • Efficiency : Maintaining compliance with documentation and billing timelines, ensuring accurate and thorough records in the electronic health record (EHR) system. Using available resources and tools to streamline workflows without compromising care quality.
