Job descriptionAssess / monitor physical, emotional, and psychological needs of patients Create hospice care plans that align with the patient's wishes and goals Direct nursing care : administering medications, treatments, and interventions Provide pain and symptom management Educate and support the patient's family and caregivers Collaborate with interdisciplinary team Maintain accurate and timely documentation Participate in on-call rotation as required by local branch Medical, Dental, Vision insurance Health Savings & Flexible Spending Accounts (up to $5,000 for childcare) Tuition discounts & reimbursement 401(k) with company match Generous PTO Mileage reimbursement Access to wellness and discount programs such as Noom, SkinIO (Virtual Skin Cancer Screening), childcare, gym memberships, pet insurance, travel and entertainment discounts and more! Schedule : Monday-Friday 8AM-5PM with On Call / Holiday Coverage Rotation As Needed
