Chronic Registered Nurse provides and coordinates clinically competent care including assessment, planning, intervention, and evaluation for assigned chronic hemodialysis patients.Delegate appropriate tasks to direct patient care staff, including RNs, LVN / LPNs, and Patient Care Technicians.Participate in decision-making, teaching, leadership, and quality improvement activities within the ESKD interdisciplinary care team.Perform ongoing, systematic patient data collection and analysis pre-, during, and post-treatment; update treatment plans accordingly.Assess and document patient and family learning needs; provide initial and ongoing education.Direct and collaborate with staff to ensure safe and effective hemodialysis treatment delivery.Administer medications per prescription or algorithm guidelines and document effectiveness.Initiate or assist with emergency response measures.Serve as a resource for clinical staff and assist in training and orienting new hires.Ensure correct collection, processing, and shipping of laboratory specimens; reschedule missed or insufficient collections.Identify expected outcomes, update assessments, and contribute to the care plan with the Interdisciplinary Team.Educate patients on transplant options and treatment modalities.Perform all additional duties as assigned by Supervisor.Physical Demands and Working Conditions
Frequent heavy lifting, patient movement, and ambulation assistance.Required bending, prolonged standing, and occasional movement of equipment up to 200 lbs with assistance.Lifting of chemical / water solutions up to 30 lbs to heights of 5 feet.Exposure to infectious diseases / materials.Work includes patient interaction, computer tasks, coordination with facility staff and physicians.Travel may be required to other locations or training sites.Must complete Ishihara Color Blindness test (accommodations considered).Supervision Requirements
May oversee Patient Care Technicians, LPNs / LVNs, and RNs as Team Leader after :Completing all FKC training requirements
At least 9 months RN experienceMinimum 3 months chronic / acute hemodialysis RN experienceEducation and Licensure
Graduate of an accredited School of Nursing.Current state RN licensure.Current CPR BLS certification.Must meet state-specific practice requirements.Required Experience
1 year nephrology nursing experience within the past 2 years.Chronic hemodialysis experience required.Required Skills
