Must be within 50 miles

Provider s own office locations- Provider must have their own office

Ash Grove- 10 days per week needed (multiple openings)

• Fort Leonard Wood- 2 days per week needed
• Kirksville- 2 days per week needed
• Cape Girardeau- 1 day per week needed
• St Joseph- 4 days per week needed

PRN as needed.

1099 compensation :

The provider will need to complete their DMA training.
