Position Responsibilities :

• Assess patient conditions, document changes, and update individualized care plans
• Educate patients and families on treatment plans and discharge instructions
• Assist with admissions, transfers, and discharges across med-surg and telemetry units
• Monitor vital signs and telemetry readings, ensuring prompt interventions
• Administer medications, treatments, and diagnostic procedures as ordered
• Collaborate with physicians, therapists, and fellow nurses to coordinate patient-centered care
