About the Role :

Join Skilled Nursing Pharmacy as a Pharmacist, where you will play a vital role in providing exceptional pharmaceutical care to our patients. With a focus on improving health outcomes, you'll collaborate with a dedicated team in a supportive environment that values your expertise.

Available Schedule :

Start times : 4 : 00pm - 6 : 00pm

Shift Differential Applicable

Responsibilities :

Review and interpret medication orders for accuracy and appropriateness.

Provide drug information and counseling to patients and healthcare professionals.

Monitor patient medication therapy for safety and efficacy.

Participate in interdisciplinary team meetings to optimize patient care.

Ensure compliance with state and federal pharmacy regulations.

Prepare and dispense medications accurately and efficiently.

Conduct medication reconciliation to prevent errors.

Educate staff on medication management best practices.

Requirements :

PharmD degree from an accredited pharmacy school.

Active California pharmacist license.

Minimum of 2 years of experience in a clinical pharmacy setting.

Strong communication and interpersonal skills.

Ability to work collaboratively in a fast-paced environment.

Knowledge of pharmacy software and technology.

Commitment to patient-centered care and safety.

Detail-oriented with excellent organizational skills.
