Responsibilities

The Supportive Care / Palliative Care Nurse Practitioner (SPCNP) works in collaboration with the HarmonyCares Medical Group to provide comprehensive palliative care services under the supervision of the Regional CMO. This role involves conducting visits for both new and existing palliative care patients and coordinating their care plans with designated staff, particularly the palliative care team. You will have the opportunity to visit patients in their homes, facilities, and hospitals, engaging in independent decision-making regarding their healthcare needs and goals.

Essential Duties and Responsibilities

  • Provide medical support and coordination for palliative care patients and their families, including :
  • Expertise in pain management, symptom control, and evaluating terminal prognoses
  • Ordering relevant therapies and tests when necessary
  • Conducting in-person and telehealth visits based on operational needs
  • Focus on enhancing patient experience and identifying individual goals of care
  • Ensure care aligns with established palliative care guidelines and standards
  • Regularly communicate with HarmonyCares Medical Group regarding new and existing patient referrals
  • Coordinate care with external primary care providers and other agencies as needed
  • Collaborate with attending physicians and internal partners on patient care
  • Perform patient visits primarily in private residences, long-term care facilities, assisted living facilities, and hospitals
  • Oversee pharmacy formulary and medication expenditures
  • Work with the Chief Medical Officer and Executive Director
  • Independently commute for patient visits and attend meetings as required

    • Qualifications

    Required Knowledge, Skills, and Experience

  • Active and unrestricted state licensure as an Advanced Practice Nurse
  • Valid NPI#, DEA, and CTP
  • Enrolled with Medicare or state Medicaid, or eligible for enrollment
  • A minimum of 3 years of experience in home-based or hospice care
