Responsibilities
The Supportive Care / Palliative Care Nurse Practitioner (SPCNP) works in collaboration with the HarmonyCares Medical Group to provide comprehensive palliative care services under the supervision of the Regional CMO. This role involves conducting visits for both new and existing palliative care patients and coordinating their care plans with designated staff, particularly the palliative care team. You will have the opportunity to visit patients in their homes, facilities, and hospitals, engaging in independent decision-making regarding their healthcare needs and goals.
