APRN / PA-C - Endocrinology (Concord Hospital - Laconia)

We are seeking a dedicated and compassionate Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our growing Endocrinology practice. This role offers the opportunity to care for patients with a wide range of endocrine disorders , including diabetes , thyroid disease , adrenal and pituitary disorders , osteoporosis , and more. A strong emphasis is placed on patient education , particularly in diabetes self-management and lifestyle interventions.

Key Responsibilities :

• Provide comprehensive evaluation, diagnosis, and treatment plans for patients with endocrine conditions.
• Manage patients with type 1 and type 2 diabetes, including insulin management and diabetes education.
• Conduct follow-up visits and monitor lab / imaging results to adjust care plans as needed.
• Collaborate with endocrinologists, dietitians, and other specialists to ensure high-quality, coordinated care.
• Educate patients and families on disease processes, medication use, and lifestyle changes.
• Participate in ongoing quality improvement and clinical development initiatives.
