Speech Therapist - Home Care PRN

About this Opportunity : We are seeking a dedicated Speech Therapist to provide evaluation and treatment in alignment with agency standards and regulations governing speech-language pathology services. Key Responsibilities :

• Arrange for specialized assessments not feasible in home environments and facilitate smooth discharge planning with appropriate community resource referrals.
• Maintain open communication with supervisors regarding rehabilitation service delivery and patient care issues.
• Contribute to established departmental goals and adhere to quality standards, safety procedures, and compliance with regulations.
• Work collaboratively with other departments and participate in multidisciplinary teams to improve service and quality.
• Communicate ongoing patient status with healthcare teams including physicians and insurance case managers.
• Perform comprehensive assessments to identify patient therapy needs and develop individualized treatment plans.
• Document patient progress following agency standards and regulatory requirements.
• Educate patients and caregivers on their condition and the functional implications, along with post-discharge care management strategies.
• Deliver speech-language pathology services according to treatment plans and professional guidelines.
• Supervise home health aides in related patient care activities.

This position offers a competitive salary range between USD $87,318.00 and USD $149,094.00 per year.
