Home Health Nurse (Roanoke)

Role Overview

This RN role focuses on providing skilled nursing care in patients' homes across all age groups. Responsibilities include :

Assessing patient needs (physical, psychosocial, environmental)

Developing and evaluating Plans of Care (POC)

Coordinating with interdisciplinary teams

Educating patients / families

Supervising HHAs / STNAs / LVNs

Managing pain and referrals

Ensuring compliance with third-party payers

Participating in on-call duties and team meetings

Required Skills & Experience

Software : Homecare Homebase Required

Documentation : OASIS Required

Licensure : Current RN license Required

Certifications : CPR Required; CHPN Preferred

Driving : Valid drivers license, reliable transportation, and insurance Required

Experience :

12 years as RN in clinical care Required

2+ years in home health Required

Education :

Required : Associate Degree in Nursing or Nursing Diploma

Preferred : Bachelor of Science in Nursing (BSN)

• Offered pay rate will be based on education, experience, and healthcare credentials.
