Description :
The Registered Nurse Case Manager develops and revises plans of treatment and discharge plans in conjunction with the patient, family, physician, care delivery team, and third-party payors.
They complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with defined time frames.
The case manager processes orders and notifies the physician of patient needs and changes in condition.
They provide an environment that promotes respect for patients, their privacy, confidentiality, and property.
The role includes initiating appropriate preventive and rehabilitative strategies and providing services that require substantial specialized nursing skills.
They consult with and educate the patient, family, and other team members regarding disease processes, self-care techniques, and preventive strategies.
The RN case manager provides supervision of LPNs to ensure patient health, safety, and compliance with the plan of care.
They direct, coordinate, evaluate, and supervise the quality of patient care services provided.
Interactions with patients, physicians, referral sources, and others should foster continued positive relationships.
The position supports a collaborative, customer-focused planning and case management philosophy designed to meet individual health and service needs while promoting quality and cost-effective outcomes.
The RN ensures compliance with state and federal legislation by completing all appropriate clinical records.
They maintain current knowledge of all available community-based services and demonstrate flexibility in visiting patients based on need rather than geography.
Problem resolution should occur at the point of contact, and the RN must ensure authorization and release of information forms are signed by the patient or responsible party.
Participation in patient care conferences about assigned patients is required.
The RN case manager also participates in the agency's operational and financial objectives by monitoring results and taking corrective actions on quality indicators, including levels of customer satisfaction.
They understand the impact of RN functions on patients, other care providers, readmissions, productivity, utilization, and revenue.
Efforts to reduce or eliminate avoidable costs and errors are essential, as is responding to findings from various quality management reviews and audits.
They maintain productivity requirements and ensure visit documentation reflects the medical necessity for care provided in the home setting.
The RN contributes to an environment of professional growth, learning, trust, and mutual respect among all employees.
They promote the recognition and respect for diversity within the workforce and participate in orientation, in-service training programs, and self-development as needed.
Commitment to professional growth and competency is expected.
The RN is responsible for key performance indicators established by their manager and performs other duties as requested.
