Provide comprehensive primary care to an established patient panel

Manage chronic conditions including diabetes, hypertension, and thyroid disorders

Adult patient care required (ages 18 and up); pediatrics optional (17 and under)

Average patient volume : 1416 patients per day

Participate in ongoing chronic care management and continuity of care Licenses and Certifications (Must Haves)

Board Certified or Board Eligible in Family Medicine

Active California Medical License (Required)

ACLS and BLS Certifications (Required)

DEA License with Schedule II authority (Required during credentialing) Work Environment

EMR System : Cerner CareB

Support Staff : Medical Assistants
