Job Description
Administer skilled nursing care for clients of all ages in their place of residence.
Coordinate care with the interdisciplinary team and assume the responsibility for coordination of care.
Assess home health and / or hospice patients to identify physical, psychosocial, and environmental needs.
Implement, develop, document, and evaluate the Plan of Care (POC).
Educate patients and their families regarding care, diet, medications, and home safety.
Provide supervision and clinical direction to other healthcare personnel to ensure quality service.
Perform on-call responsibilities and provide communication to patients, families, and team members.
Critical Information
Weekend on-call required about every four weeks.
Coverage area includes Vancouver, WA proper and surrounding towns.
Full-time availability is not guaranteed.
Education / Licenses Needed
Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program.
Current state license as a Registered Nurse.
CHPN certification preferred.
Proof of current CPR certification required.
1-2 years of experience as a Registered Nurse in a clinical care setting.
2+ years of home health experience required.
