Shift Time : 3 : 00 PM – 11 : 30 PM

Position Overview

We are seeking a dedicated Licensed Practical Nurse (LPN) to join our Long-Term Care (LTC) team for a 16-week travel assignment in Malone, NY. The ideal candidate will have strong clinical experience in LTC settings, excellent communication skills, and a commitment to providing high-quality patient care.

Daily Responsibilities

Deliver safe and effective nursing care to LTC residents

Administer medications and treatments per provider orders

Monitor, document, and report changes in patient condition

Collaborate with RNs, CNAs, and the interdisciplinary care team

Participate in care planning, progress notes, and patient assessments

Maintain compliance with facility policies, infection control standards, and regulatory guidelines

Support CNA functions when necessary, including assistance with ADLs

Utilize facility EMR for accurate and timely charting

Required Qualifications

Active LPN License – New York

Minimum 1 year of recent LTC experience

AHA BLS Certification (must be valid and active)

Must be comfortable covering CNA-level tasks as needed

Strong understanding of LTC regulations, resident rights, and standard protocols

Ability to work evening shifts consistently
