As Care Coordination Nurse, you'll contribute to our success in the following ways :
Conduct in-home and facility-based assessments to determine patient needs and appropriate levels of care
Develop individualized care plans that reflect each patient's clinical condition, personal goals, and regulatory requirements
Coordinate services across Personal Care, Skilled Home Health, and Hospice to ensure continuity and consistency of care
Collaborate with interdisciplinary team members including physicians, therapists, social workers, and personal care coordinators to support holistic, patient-centered care
Educate patients and families about available services, care options, and community resources
Maintain accurate, timely, and compliant documentation within the electronic health record system
Monitor patient progress, reassess needs, and coordinate transitions between service lines as conditions change
Communicate proactively with referral sources and internal teams to ensure timely admissions and smooth onboarding
Participate in case conferences and care coordination meetings to support comprehensive care planning
Serve as a wellness advocate by promoting preventive health, safety, and patient empowerment
Ensure accurate physician orders are signed and received timely
Facilitate timely provider visits, communication between the agency, patients, providers and facilities
Performs other duties / projects as assigned.
What is Required?
Current, unrestricted RN license
3+ years experience of clinical nursing is required.
2+ years experience in home health or hospice with experience driving patient care outcomes, supporting quality initiatives or partnering with leaders to resolve care concerns and ensuring compliance.
Excellent verbal and written communication skills
Must have a dependable vehicle, a valid driver's license, and current auto insurance in accordance with the laws of the State
Ability to travel to daily to patient homes, assisted living, and skilled nursing facilities within the assigned territory.
