About The Position

Responsibilities

• Evaluate residents' physical, emotional, and functional status to determine appropriate therapy needs.
• Develop and implement individualized treatment plans to improve mobility, strength, and independence in daily activities.
• Provide therapeutic interventions and adaptive techniques to help residents achieve rehabilitation goals.
• Collaborate with an interdisciplinary team including PTs, SLPs, nursing staff, and physicians to ensure comprehensive care.
• Document evaluations, progress notes, and discharge summaries in accordance with state and facility requirements.
• Educate residents, families, and caregivers on home programs, adaptive equipment, and continued therapy needs.
• Ensure compliance with all applicable federal, state, and facility regulations and standards of care.

Graduate of an accredited Occupational Therapy program. Prior experience in a skilled nursing facility (SNF) or similar setting preferred. Strong interpersonal and communication skills. Compassionate, patient-focused approach to care.
