Job Opening : Clinical Coordinator (LPN) Location : Fairburn, GA Schedule : Monday–Friday, 8 : 00 AM – 6 : 00 PM Monday- Friday 10 hours Rotaating Weekends Satturday and Sunday 8 : 00am - 4 : 00pm Employment Type : Full-Time Reports To : Registered Nurse (RN) About the Role : We are seeking a highly experienced and motivated Licensed Practical Nurse (LPN) to join our leadership team as a Clinical Coordinator. This role is ideal for an LPN with a strong clinical foundation and a desire to grow into the operational and business side of homecare—particularly within the framework of DBHDD services. Key Responsibilities : Assist with clinical coordination and office management under RN supervision Support case management and contribute to the development and implementation of individualized care plans Complete and update HRST assessments, protocols, and documentation Provide clinical support and guidance to field nurses and direct care staff Participate in leadership functions, including staff training, quality assurance, and compliance monitoring Collaborate with interdisciplinary teams to ensure continuity and quality of care Qualifications : Active LPN license in the state of [Insert State] Minimum 4 years of LPN experience, including roles such as Unit Manager, ADON, or similar leadership positions Background in homecare, preferably with experience in IDD / DD populations Familiarity with DBHDD regulations, HRST tools, and person-centered planning.
