Job Details
Prescribes coordinates and delivers care through the nursing process. Maintains the standards of professional nursing. Supervises CNA and coordinates care with other health care workers as assigned.Provides comprehensive initial assessments to all clients and on subsequent visits continually assesses Physical, Psychosocial, Environmental and community resources available utilizing the principles of growth and development over the life spanInitiates Plan of Care, re-evaluates the need for care and modifies Plan of Care as ordered / needed. Establishes discharge plans with patient, physician, other disciplines and caregivers.Completes OASIS dataProvides patient care using current Federal, State and Agency regulationsAssesses educational needs of patient, families, caregivers and implements appropriate teaching strategiesCompletes documentation according to regulatory guidelines.Promotes a clean and safe environment of care, utilizing the SAFE error prevention habitsProvides the highest standard of privacy and confidentiality in matters involving patients, coworkers, and the hospital by abiding by the Standards of Conduct
