Job Summary :

• As an After-Hours Care Coordinator, you’ll play a vital role in maintaining seamless care and service delivery outside of standard business hours.
• You’ll serve as the primary point of contact for clients, caregivers, referral sources, and internal teams during the evening and weekend hours.
• In this dynamic role, you’ll be responsible for re-staffing caregiver callouts, managing schedule changes, staffing new cases, and handling any urgent issues or escalations that arise.

You’ll help ensure that every client’s care experience remains consistent, responsive, and well-coordinated—no matter the hour.
