POSITION SUMMARY

Meet with patients and families to explain the scope of hospice services, coordinate admission of patients and ongoing plan of care.

ESSENTIAL FUNCTIONS

• Assess patient's physical condition according to Plan of Care; determine and / or revise plan of care regarding medications and DME.
• Provide emotional support to patients and families while instructing them in activities of daily living.
• Complete daily paperwork and charting, thoroughly and accurately according to Policy.
• Evaluate the patient for hospice appropriateness and complete admission visit and documentation as needed; communicate with the Case Manager on significant findings.
