Role Overview
- Conduct in-depth adult and geriatric assessments that include medical history, diagnoses, health education, and care management referrals, providing essential insights for optimal patient care.
- Perform and order necessary point-of-care and lab-based testing.
- Collaborate with a multidisciplinary team, including physicians, case managers, and family members to ensure comprehensive care.
- Provide services primarily in members' homes or Skilled Nursing Facilities, emphasizing personalized and culturally competent care.
What We Offer
Work-Life Balance : Enjoy a consistent schedule, typically Monday to Friday, from 8 : 00 AM to 5 : 30 PM.Competitive Salary : Base compensation of $105,500 - $168,400, based on experience and market analysis, along with uncapped quarterly incentive opportunities.Comprehensive Benefits Package : Includes mileage reimbursement, health insurance, retirement savings plans, paid holidays, and generous PTO accrual.Professional Development : Access to continuing education opportunities, mentorship, and support for obtaining additional state licenses as needed.Your Qualifications
Master's Degree or equivalent experience with successful NP or PA licensure.Current NP / PA licensure in the state of practice, including prescription authority.Board certification in relevant specialties (e.g., Adult, Family or Acute Care) is preferred.Current BLS, ACLS or CPR certification.A minimum of one year of experience as a Nurse Practitioner or Physician Assistant.Willingness to travel up to 80-90%, including overnight and weekend travel.Strong verbal and written communication skills coupled with proficiency in medical software.The Matrix Culture
