Role Overview

• Conduct in-depth adult and geriatric assessments that include medical history, diagnoses, health education, and care management referrals, providing essential insights for optimal patient care.
• Perform and order necessary point-of-care and lab-based testing.
• Collaborate with a multidisciplinary team, including physicians, case managers, and family members to ensure comprehensive care.
• Provide services primarily in members' homes or Skilled Nursing Facilities, emphasizing personalized and culturally competent care.
