Job Summary

The home health registered nurse plans, organizes, and directs home care services with a focus on community health education and experience. You will work closely with individuals and families within their homes and communities, utilizing available resources to meet their unique needs.

Duties & Responsibilities

Patient Care

• Develop and initiate personalized care plans in partnership with patients, representatives, and caregivers.
• Implement preventative and rehabilitative nursing procedures as ordered by physicians.
• Provide counseling and education to patients, caregivers, and families.
• Prepare clinical notes and maintain effective communication with the healthcare team.
• Identify discharge planning needs and implement appropriate measures.
