LPN _ Assisted Living, Memory Care, Skill Nursing

Duties & Responsibilities

• Provide compassionate, high-quality nursing care to assigned patients
• Administer and document medications and treatments according to clinical standards
• Educate and support patients and their families regarding care plans and health needs
• Collaborate with interdisciplinary team members to plan, deliver, and evaluate care
• Assist with patient admission, transfer, and discharge processes
• Monitor and report changes in patient condition, including vital signs and symptoms
• Accurately document care activities and patient responses in the medical record
• Maintain a clean, safe, and therapeutic environment for patients
• Support daily unit operations and assume charge duties when assigned
