Job descriptionLocal travel within the Pueblo area Mileage reimbursement at $0.45 / mile Reliable transportation and valid car insurance required Active Colorado or Multi-State RN License (compact license accepted) Preferred : HomeCare HomeBase (HCHB) and Hospice experience Conduct comprehensive assessments for assigned patients, identifying clinical and non-clinical needs. Develop and implement individualized care plans with measurable goals and interventions. Collaborate with primary care providers and other healthcare professionals to coordinate ongoing care. Monitor patient progress and modify care plans as needed to achieve desired outcomes. Facilitate care transitions and discharge planning in collaboration with physicians, social workers, and other stakeholders. Identify opportunities for health promotion and illness prevention. Maintain detailed documentation of patient assessments, interventions, and outcomes. Act as a patient advocate by ensuring privacy, dignity, and confidentiality in all interactions. Associate’s or Bachelor’s Degree in Nursing (BSN preferred). Current, unrestricted RN license (Colorado or Compact State). Case Management certification preferred. Strong analytical, problem-solving, and critical thinking skills. Ability to manage multiple cases while maintaining attention to detail. Excellent communication and organizational abilities.
