Key Responsibilities
Provide mobile outreach, assessment, and minor treatment to individuals experiencing homelessness across Bucks County-services may be delivered in a mobile unit, shelter, encampment, or drop-in center.Conduct comprehensive nursing assessments, including health history, vital signs, and point-of-care testing.Offer education and counseling on self-care, harm reduction, medication adherence, and preventive health.Provide minor medical treatments (e.g., wound care) and assist clients in accessing primary and specialty care.Identify and document health concerns, risks, and goals using trauma-informed and motivational interviewing techniques.Collaborate with interdisciplinary care teams-mental health, substance use, case management, and outreach-to coordinate holistic care.Facilitate connections to Medication-Assisted Treatment (MAT) and other evidence-based services for substance use disorders.Support clients in navigating the healthcare system, following through on referrals, and maintaining continuity of care.Participate in maintaining and restocking mobile unit equipment and supplies; ensure safety and cleanliness.Maintain accurate, timely documentation in electronic health records and outcomes tracking systems.Qualifications
Education : Licensed Practical Nurse (LPN), Registered Nurse (RN), or Certified Registered Nurse Practitioner (CRNP). If CRNP, a DATA 2000 waiver is required.Experience : Minimum two years of clinical experience with a focus on community or public health.Preferred Skills and Knowledge :Experience serving individuals with substance use disorders, mental illness, or complex medical conditions.
Passion for harm reduction and population health principles.Experience working with individuals experiencing homelessness.Bilingual (English / Spanish) highly desirable.Familiarity with electronic health records and documentation standards.Other Requirements :Valid driver's license, safe driving record, and proof of insurance.
