New York Licensed Clinical Pharmacy Specialist

A company is looking for a Lead Clinical Pharmacy Specialist to support pharmacy quality programs and medication management initiatives.

Key Responsibilities

Conduct patient medication history reviews and identify barriers to care

Deliver Medication Therapy Management services and coordinate medication reconciliation

Collaborate with prescribers and interdisciplinary teams to optimize medication therapy and improve clinical outcomes

Required Qualifications, Training, and Education

Bachelor's Degree and PharmD from an accredited college of pharmacy

Current, unrestricted Pharmacist License in the respective state

Completion of an MTM training program preferred

10+ years of relevant professional work experience, with 3-5 years in a specific position-related role

Experience with CMS Comprehensive Medication Reviews and managed care
