At Geriatric Specialty Care of Reno, we're expanding our model for caring for seniors that provides high-quality, comprehensive in-home healthcare services to seniors all across Northern Nevada. From private residences and long-term care to assisted living facilities, our team of nurses, physicians, advanced practice providers, and care providers visit patients where they're most comfortable—creating a seamless, convenient, and personalized process for seniors and their caretakers.

The Role

The Provider is responsible for the diagnostic evaluation, management, and therapeutic health care of primary care patients of PPH in our Reno Clinic location as well as patients residing in assisted living communities, and residential care homes. This care includes-

• Obtain history and physical on new patients
• Order / interpret diagnostic tests
• Diagnose and treat chronic conditions and episodic illness
• Perform Advance Care Planning
• Accurate and timely documentation in the EHR
• Coordinates with other members of the care team, both internal and external partners

In addition to the above, the Provider is responsible for the annual comprehensive health assessments in accordance with health plan partners and Medicare guidelines. The assessments may be done in the clinic or residential setting. This service includes :
