A company is looking for a Memory Care Coordinator (Hybrid).

Key Responsibilities

Coordinate care and support for caregivers, developing individualized care plans and connecting them to resources

Maintain ongoing communication with care partners and healthcare teams to ensure comprehensive support and documentation

Advocate for care partners by providing education, emotional support, and access to community resources

Required Qualifications

Experience working with individuals diagnosed with dementia or memory-related conditions

Familiarity with basic healthcare terminology and local resources for dementia care

Comfortable with digital communication and proficient in electronic medical records (EMR)

Demonstrated empathy and emotional intelligence in caregiving interactions

Ability to manage multiple cases efficiently in a remote work environment
