Patient Care Coordinator, HHA Manager

VNS Westchester, A Destination Employer,  has an exciting opportunity for a Patient Care Coordinator in our White Plains, NY headquarters! We are the premier Home Health organization in the Hudson Valley Region.

The Patient Care Coordinator is responsible for managing various aspects of patient care. The intake process, ensuring a smooth and efficient communication and flow of information while providing excellent customer service. Oversight of daily operations of home health aide services such as supervision of home health aides, ensuring Electronic Visit Verification (EVV) compliance, initiating discharges, scheduling / coordination of home health aide services and collaborating with clinicians and families to ensure that clients receive timely and appropriate care in their homes.

Key Responsibilities :

Intake :

  • Address patient inquiries and concerns with empathy and professionalism.
  • Provide clear information regarding services and the intake process, including required documentation and procedures.
  • Collect and verify patient information, including medical history, demographic details, and insurance / payment information.
  • Enter patient information accurately and in a timely manner into electronic health records (EHR) systems.
  • Collaborate closely with nursing staff and administrative teams to ensure an efficient intake process and start of services.
  • Identify and resolve issues related to patient intake, such as discrepancies in information or scheduling conflicts.
  • Provide support to patients in navigating the healthcare system.
  • Maintain patient confidentiality and adhere to HIPAA regulations.

Oversight of daily operations of home health aide services :

  • Schedule and coordinate home health aide assignments based on client needs, staff availability, and care plans.
  • Communicate effectively with home health aides, clients, and families to confirm appointments and address any scheduling concerns.
  • Monitor and adjust schedules to accommodate changes in client needs or staff availability, including managing last-minute requests or cancellations.
  • Maintain accurate records of scheduling activities, client care plans, and staff hours in the EHR.
  • Collaborate with clinical and administrative team to ensure continuity of care and alignment with treatment plans.
  • Assist in the onboarding and training of new home health aides, ensuring they understand scheduling protocols and client needs.
  • Act as a point of contact for home health aides to address questions, concerns, and feedback related to their schedules.
  • Ensure compliance with regulatory requirements and company policies regarding scheduling and client care.
  • Analyze scheduling patterns to improve efficiency and maximize resource utilization.
  • Oversee the implementation and utilization of EVV systems within the home health agency.
  • Ensure that home health aide visits are accurately recorded and verified using electronic systems on a daily basis.
  • Assist aides with the technology, and ensure that patient care is documented properly and efficiently.
  • Ensure accurate and timely entry of visit data, including start and end times, patient interactions, and services provided.
  • Ensure that discharge summaries are initiated promptly upon a patients discharge from a home care service and primary care provider is notified within 24-48 hours.
  • Ensure that the discharge summary is included in the patients electronic health record (EHR).
  • Maintain accurate records of communication with patients, family and home health aides including complaints and incidents.
  • Participate in quality improvement initiatives.
  • Qualifications :

  • High school diploma or equivalent; an associate degree in healthcare administration or related field is preferred.
  • Previous experience in scheduling, coordination, or administration within a healthcare setting is desirable.
  • Familiarity with home health care services and understanding of client needs.
  • Strong organizational skills and attention to detail, with the ability to manage multiple priorities.
  • Excellent communication and interpersonal skills, with a focus on customer service.
  • Proficient in HHA exchange scheduling software and Microsoft Office Suite.
  • Ability to work independently and as part of a team.
  • Availability to be on-call every other week.
  • This is a Hybrid Position.

    Salary : $24-$26 per hour
