Description :

Monday to Friday from 8 : 00 AM to 4 : 30 PM with weekend rotation of one out of four weekends required.

Responsible for providing skilled care and case management to patients and families in accordance with the State Nurse Practice Act.

Adheres to accepted professional standards of practice.

Utilizes nursing theory and processes in the provision of services.

Communicates and documents observations and assessments effectively.

Promotes and maintains health through direct care, teaching, counseling, and appropriate rehabilitative measures.
