Busy New York Hospital is seeking a Care Management Associate to join their team. Responsibilities include : Handle incoming / outgoing calls on the MLTC / PCS ACD. Schedule appointments for Care Management staff and assist members with scheduling medical visits. Maintain member files, track membership status, and support daily workflow. Serve as a liaison between MetroPlus and HHA / PCA vendors regarding member services. Enter and process authorizations in CareConnect, including prior approvals and service updates. Monitor authorization queues to ensure timely processing. Track member hospital / ER admissions, nursing home placements, and other key outcomes. Coordinate required documentation such as Doctor’s Orders and M11Qs. Print and mail Plans of Care and track member absences. Notify Care Managers when members cannot be reached and resolve missing or conflicting documentation. Complete additional tasks as assigned by leadership. Qualifications : High School Diploma / GED required. At least 1 year of relevant experience; managed care or call center experience preferred. Medical terminology and ICD-9 / CPT-4 knowledge preferred. Strong communication, customer service, and organizational skills. Proficiency in Microsoft Office.
