Family Nurse Practitioner (FNP) | 100% Outpatient | Patient-Centered Model of Care

A well-established independent primary care practice in Shallotte, North Carolina is seeking an experienced Family Nurse Practitioner (FNP) to join our collaborative healthcare team. Our practice follows the Patient-Centered Medical Home (PCMH) model of care — providing coordinated, comprehensive, and preventive healthcare that focuses on long-term patient relationships and whole-person wellness. Position Highlights

Full-time, 100% outpatient family medicine role
Work alongside a team of experienced team of providers
Focus on preventive care, chronic disease management, and patient education
Manage acute and routine primary care visits
State-of-the-art facility equipped for in-office procedures, digital x-rays, and lab testing
Support from an experienced clinical and administrative team

Responsibilities

Provide comprehensive primary care services for patients across the lifespan

Perform physical exams, diagnose and manage acute and chronic conditions

Order and interpret diagnostic tests, prescribe medications, and develop care plans

Promote wellness, disease prevention, and patient education

Collaborate closely with physicians and care teams to ensure continuity of care

Document all encounters accurately in the Electronic Health Record (EHR) system

Participate in quality improvement initiatives consistent with PCMH standards

Qualifications

Current licensure as a Family Nurse Practitioner in the State of North Carolina (or eligibility)

Graduate of an accredited Nurse Practitioner program

Minimum of 1–2 years of primary care experience preferred

Strong clinical judgment, communication, and interpersonal skills

Commitment to patient-centered, evidence-based care

Proficiency with EHR systems (Athenahealth, Epic, or similar platforms)
