Overview

Pay range :   $33 / hr. - $43 / hr. based on experience

Hospice nurses provide end of life care to help ensure the comfort and quality of life for their patients.  They primarily work in collaborative, team-driven environments that require frequent communication with their patients, families and other caregivers.  The ideal candidate should be detail-oriented, patient and calm.  Hospice nurses are caring and kind individuals who possess a special hospice heart who can help our patients celebrate the rest of their life’s journey.

Responsibilities

Hours of operation :   7am - 7 : 30pm and every other weekend

Work Territory :   Boca Raton / Boynton Beach / West Palm Beach

Duties and Responsibilities

• Coordinate the care for the patient / family under the direction of the primary physician
• Conduct assessment of the patient / family needs
• Develop the IDG (Interdisciplinary Group) Care Plan
• Ensure the implementation and coordination of the interventions and services described in the care plan.
• Pledge to make the comfort and care of patients and families the highest priority
• Consistently provide a high level of personalized, sincere and engaging service to patients and families at all times
• Ensure the complete satisfaction of patients and families; promptly and effectively address all concerns.
