In this position, you will collaborate with medical professionals to deliver the best care for your patients. You will also conduct initial home visits, assess patient eligibility for services, and create tailored care plans.\n\nQualifications : \n
• Graduation from an accredited nursing program (diploma, Associate, or Baccalaureate).\n
• Current state license as a Registered Nurse.\n
• Valid state Driver's License.\n
• Minimum one year of RN experience; two years preferred.\n
• Proof of current CPR certification and Hepatitis consent / declination.\n
