About The Position

Nurse Practitioner - Primary Care Clinic

Join a dedicated healthcare team as a Nurse Practitioner in a primary care outpatient clinic, focusing on providing value-based care to adults on Medicare. This permanent position in Portsmouth VA offers the opportunity to work collaboratively with a dedicated care team to ensure patients receive comprehensive and coordinated healthcare services.

Employment Type : Permanent
Hours Per Week : 40
Schedule : Monday - Friday, 8 AM - 5 PM
Salary : $105,000 - $135,000 / year (Depending on Experience)

Job Requirements

Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate

National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC

Active, non-probationary state Nurse Practitioner license

Active DEA license

US Work Authorization

At least 1 year of experience in primary care : internal medicine, geriatrics, or family medicine preferred

Responsibilities

Provide exceptional primary care in a clinical setting

Conduct office visits for routine and acute issues

Administer annual wellness visits and health risk assessments

Coordinate care with other healthcare providers, specialists, and facilities

Lead population health initiatives in coordination with the Care Team

Assist with phone triage and patient outreach

Educate patients on health conditions, care plans, and treatments

Participate in promotional activities and conduct home visits as needed
