The Afterhours Care Manager Registered Nurse plays a crucial role in assessing and coordinating the needs of patients and their families during after-hours care. This position involves maintaining primary control and professional management of each patient assigned while ensuring high-quality care in accordance with their individual plans.
- Addressing patient needs through routine visits, admissions, death visits, on-call inquiries, and any other assigned duties in line with the care plan.
- Providing after-hours coverage for designated teams and functioning as part of an interdisciplinary team, ensuring seamless communication between physicians, patients / families, and the hospice team.
- Completing clinical administrative tasks, including workflow auditing and preparing for surveys, alongside other assigned responsibilities.
About You : To excel in this position, you should be capable of effectively meeting each essential duty. The following qualifications are required :
Current RN licensure in the state where you practice.Graduation from an accredited nursing program.A minimum of one year of RN experience in acute care, oncology, long-term care, hospice, or home health.State-specific requirements, if applicable, will take precedence.Licenses, Certifications, and Registration :
Valid nursing license in the state of employment.CPR Certification (must be completed in person).Reliable vehicle for work purposes, with current automobile insurance and a valid driver’s license.Equipment / Tools / Work-Aids :
Proficient in using nursing assessment tools, computers, and communication devices.Access to reliable transportation for work-related tasks.Specialized Knowledge / Skills :
Understanding hospice principles and practices, including industry standards and regulations.Skillful in nursing practice with the capacity to perform thorough patient assessments.Adept in recognizing and addressing the unique needs of terminally ill patients and their families.Knowledge of community resources and hospice service providers.Ability to prioritize tasks effectively in a dynamic healthcare environment.Strong collaboration skills to facilitate cooperation across operations, clinical, and business teams for improved patient outcomes.Excellent communication abilities, both oral and written, to interact with individuals from diverse backgrounds.Commitment to confidentiality and ethical practices in all professional interactions.Familiarity with technology, including mobile devices and Microsoft Office applications.Proven accountability and leadership skills for supervising, training, and evaluating team members.Capacity to manage program operations to assure quality and compliance.Personal Traits and Qualities :
