Reports To : Practice Manager
This position involves providing individual counseling, conducting needs assessments, and developing and implementing personalized treatment plans. The clinician is responsible for maintaining accurate and confidential client records, offering crisis intervention, and collaborating with other professionals to coordinate comprehensive care. The role requires experience in a clinical or counseling setting, knowledge of therapeutic modalities and evidence-based practices, and strong assessment, problem-solving, and communication skills. Services provided are, but not limited to : psychosocial assessment, treatment planning, consultation, crisis intervention, education, collaboration, information and referral, coordination of services, advocacy.
Must be able to occasionally work more than 40 hours per week.
Education
Master's Degree in Social Work.
One to two years related experience perferred.
Proficient in Microsoft Office, email and basic computer skills for other software used throughout the hospital for your area of responsibility.
Must be a Social Worker licensed by the State of Indiana, or eligible to take exam and pass within 6 months of hire.
Should take no longer than 3 months to become proficient in this position. Knowledge of changes in regulations and requirements. Knowledge of healthcare delivery system, medical terminology, screening material, assessment skills, patient safety and quality initiative, management and treatment protocols and regulatory guidelines. Reading and math skills to analyze data, calculate results, and prepare graphs. This position requires frequent standing, walking, sitting, lifting and carrying 0-10 lbs and pushing and pulling 0-12 lbs.
