Are you an experienced Registered Nurse looking to make a real difference in the lives of patients? At Presbyterian Complete Care Home Health , we are on the lookout for passionate individuals to join our dynamic team. In this vital role, you will be responsible for the case management of Home Healthcare patients, ensuring that care delivery meets quality and utilization guidelines while achieving desired clinical outcomes.

Your Role as a Case Manager

As a Case Manager , you will :

  • Ensure the completeness, timeliness, and appropriateness of patient care plans.
  • Communicate effectively with a multidisciplinary team and community resources to achieve care goals efficiently.
  • Document care accurately and ensure it is reimbursable.
  • Act as a patient advocate and mentor newly hired nurses.
  • Manage your daily assignments and follow up on patient care.

  • Qualifications

    To be successful in this role, you should have :

  • Associate Degree in Nursing.
  • State of New Mexico or Compact State Nursing License.
  • BLS certification (required at time of hire).
  • ACLS and PALS certifications.
  • Minimum one year of acute care experience within the last 3 years; hospice or home care experience preferred.
  • Your Responsibilities

    As a Case Manager, you will :

  • Demonstrate comprehensive nursing care management through timely assessments and interventions.
  • Coordinate care plans with a multidisciplinary team and community resources.
  • Ensure compliance with documentation regulations.
  • Maintain productivity standards for visits and caseload management.
  • Precept newly hired nurses and serve as a professional role model.
  • Participate in continuous quality improvement activities.
  • Be responsible for weekend and holiday coverage as needed.
