The Home Therapy Registered Nurse is responsible for delivering high-quality, patient-centered care to individuals undergoing home dialysis therapy. As a key member of the End Stage Kidney Disease (ESKD) team, you will assess, plan, and manage treatments while educating patients and caregivers on safe home dialysis procedures. This role combines clinical expertise, teaching, leadership, and care coordination to improve health outcomes and empower patients to manage their therapy independently.
Key Responsibilities :
Deliver clinically competent home dialysis care , including planning, intervention, and evaluation.Monitor and document patients' response to dialysis training and therapy.Educate patients and families on dialysis equipment, procedures, and home safety.Collaborate with interdisciplinary team members and communicate with physicians as needed.Conduct home assessments for candidate suitability and environment safety.Administer prescribed medications and respond to emergencies when needed.Ensure compliance with lab collection and shipment procedures.Assist with the training and onboarding of new staff.Provide leadership and oversight to LPNs, RNs, and support staff when designated.Participate in on-call, weekend, night, and holiday rotations as required.Travel to patient homes and training sites as necessary.Required Experience :
Graduate of an accredited School of NursingActive RN license in MassachusettsCurrent CPR / BLS certificationAt least 1 year of nephrology nursing experience in the past 2 yearsMinimum of 1 year as a Home Dialysis RNNice-to-Have Experience :
Peritoneal dialysis and / or home hemodialysis trainingSupervisory experience or Team Lead responsibilitiesFamiliarity with chronic disease educationRequired Skills :
Clinical dialysis experience in a home settingStrong teaching and patient education skillsAbility to lift / move equipment up to 200 lbs with assistancePreferred Skills :
Experience training home care partnersComfort with rotating shifts and travelFamiliarity with electronic health records and documentation protocolsAbility to provide emergency response supportStrong collaboration and team communicationAdditional Skills :
