MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

About the Role

Aging Well Adult Day Care, part of Sevita, is excited to hire a caring and dedicated LPN ! We're looking for someone reliable, motivated, and confident in their nursing skills. This role offers meaningful work, variety in your day, and a supportive team environment.

Responsibilities

Monitor health conditions and ensure quality care

Administer medications and document accurately

Oversee staff involved in medication administration

Complete assessments and update Individual Service Plans (ISPs)

Train staff on infection control, medication administration, and safety

Coordinate with external medical providers and advocate for individuals

Provide education on treatments, self-medication, STD prevention, smoking cessation, and more

Qualifications

1+ year nursing experience with the population served

Current MA LPN license

Strong communication and teamwork skills
