Call Requirement : None

Weekend Requirement : Every other weekend (with a weekday off during rotation week)

Coverage Area : Van Buren, Calhoun & Kalamazoo Counties

Required Qualifications

Position Summary

The Physical Therapist evaluates, plans, and administers medically prescribed physical therapy for patients with injuries or conditions affecting muscles, nerves, joints, and bones. The focus is to improve functional ability, increase strength, manage pain, and support functional independence. Must demonstrate competencies specific to the population served.

Physical Therapist (PT) – Home Health – Travel / Contract\n\nShift : 8 : 00 AM – 5 : 00 PM\nHours : 40 hrs / week \nCall Requirement : None\nWeekend Requirement : Every other weekend (with a weekday off during rotation week)\nFloat Requirement : No\nCoverage Area : Van Buren, Calhoun & Kalamazoo Counties\n\nInterview : Leader will call to interview\n\nRequired Qualifications\n\nActive Michigan PT license\n\nBLS (AHA)\n\nHome Health experience required\n\nValid Michigan driver’s license, personal vehicle for daily business use, and proof of insurance (required for Bronson at Home)\n\nPosition Summary\n\nThe Physical Therapist evaluates, plans, and administers medically prescribed physical therapy for patients with injuries or conditions affecting muscles, nerves, joints, and bones. The focus is to improve functional ability, increase strength, manage pain, and support functional independence. Must demonstrate competencies specific to the population served.\n\nClinical Responsibilities\n\nInterpret and implement referrals\n\nPerform standardized assessments to evaluate ROM, strength, coordination, and functional abilities\n\nEstablish functional baselines and track progress\n\nParticipate in interdisciplinary discussions, rounds, and clinics\n\nDevelop education and treatment plans\n\nProvide patient, family, and community education\n\nSet patient care goals and estimate treatment duration\n\nDeliver direct patient care (individual and / or group PT treatments)\n\nInstruct patients in skills needed for functional improvement\n\nMay design / fabricate splints and assistive devices\n\nContribute to program development and department process improvement\n\nServe as a resource and provide in-services within the Bronson Rehab System\n\nMay cross-train and provide coverage across other Rehabilitation Services as needed\n\nAdministrative Responsibilities\n\nDocument evaluations, treatment plans, interventions, and progress according to compliance timelines\n\nMaintain accurate, legible, and complete electronic health records\n\nCommunicate any limitations affecting timely documentation\n\nEnsure documentation reflects patient status and adjustments to treatment plans
