As a Home Health Physical
Therapist, here’s a big-picture view of what you’ll
do :

Provide home-based physical therapy to
patients who have lost mobility, strength and functionality due to
an illness or injury

Work as part of a home health team which
may include an RN, LPN, CNA, HHA, OT and SLP, focused on the
patient’s plan of care and goals

Observe movements, conduct assessments,
document progress and modify care plan as
