Job Description

The Behavioral Health Clinician position in our Collaborative Care Program serves as an integrated member of the team at our Internal Medicine Faculty Associates-Primary Care practice in Morristown. The position provides brief evidenced-based focused interventions in collaboration with the patient's primary medical provider and the program's consulting psychiatrist, as part of an interdisciplinary treatment approach. The position allows for patients to receive the care they need within a familiar setting.

Job Functions :

01. Establishes rapport with patients through active listening, empathy, positive regard, and non-judgmental communication.

02. Screens, assesses, and monitors patients for common mental health and substance use disorders.  Facilitates patient engagement and follow-up care.

03. Educates patients and families on diagnoses, treatment options, relapse prevention, and self-management strategies.

04. Provides brief, evidence-based behavioral interventions such as behavioral activation, cognitive behavior therapy, problem-solving treatment, motivational interviewing, or other treatments as appropriate.

05. Demonstrates an understanding of patient problems / dynamics.  Identifies defenses, schemas, attitudes, behaviors, and coping strategies and applies appropriate interventions based on evidence-based best practices.  Demonstrates cultural competence and age-specific clinical skills.

06. Collaborates closely with medical providers, psychiatric consultants, and other specialty providers on a shared plan of care.

07. Facilitates in-clinic and community referrals to evidence-based psychosocial treatments and social services as appropriate.

08. Develops, implements, and revises treatment goals in collaboration with medical providers and psychiatric consultant, using clinical data and validated screening measures during regularly scheduled case consultations.

09. Tracks treatment response, patient progress, and safety issues systematically through scheduled visits, phone follow-ups, and caseload data using a clinical registry.

10. Identifies and facilitates treatment plan changes for patients who are not improving and may need more intensive or specialized services.

11. Collaborates with patients nearing discharge to create relapse prevention and self-management plans.

12. Completes accurate, timely, and legible documentation of patient contacts / interactions in the electronic medical record.  References standardized scales, treatment goals, and identified safety issues in documentation.   Maintains complete and compliant medical records, including charge tickets, schedules, and other relevant information for quality assurance.

13. Ensures confidentiality of patients and medical records, adhering to all regulatory, administrative, and organizational requirements.

14. Meets designated productivity standards while contributing to program quality improvement and clinical development initiatives.

15. Actively collaborates with medical practice team(s) to build effective relationships, promote service utilization, and provides in-service education as needed to support program awareness / impact and support positive patient outcomes.

16. Other tasks and responsibilities as required.

Required :

• Education and / or Experience : Master’s Degree in a mental health discipline, LCSW or LPC required.
• Previous clinical or counseling experience.
