$750.00 Sign-on bonus. Located in Keene and surrounding towns

SUMMARY

The Residential and Community Participation Services (CPS) Float Staff position is responsible for providing support in residential and community participation service program for individuals with developmental disabilities or acquired brain disorders. Duties and responsibilities vary in intensity and frequency based upon the individuals and may include personal care or working with someone with a behavior support plan. The position is expected to work a flexible schedule including days, nights, weekends and overnights. Schedule is variable and changes every two weeks.

ESSENTIAL FUNCTIONS

• Assists the individual to develop and maintain independent living skills in areas including, but not limited to; social skills, cooking and nutrition, communication, money management, community integration, household duties, safety, health and personal hygiene.
• Provides medication administration, if the individual is unable to administer their own medications. This is determined by the agency nurse.
• Supports individual to achieve the goals outlined in their individual service agreement and behavior plan.
• Communicates with the Program Manager, CSS Director, and MDS Nurse(s) any concerns impacting the individual's well-being, injury, illness or changes in emotional and / or physical status in a timely manner.
• Completion and timely submission of all required including but not limited to : daily notes, medication logs, incident reports, progress notes, mileage reimbursement forms, seizure logs, weight, in a timely manner.
• Ability to work with a diverse group of people, including staff, individuals, guardians, families, co-workers, and management.
• Foster and assist individual's good relationships with family, friends, neighbors and the community at large to promote better living standard for individuals.
• Provides transportation to the individual for appointments, community activities etc.
• Follows safety protocols to foster a safe working environment.
• Reports unsafe conditions, environment, and equipment to the Program Manager.
• Adheres to all relevant policies and procedures
• Maintains individual's confidentiality.
• Recognizes and acts on the legal responsibilities concerning the safety and welfare of the individual.
• Ability to communicate effectively both verbally and in written form.

EDUCATION / EXPERIENCE

Minimum high school diploma or equivalent. Ideal candidate has one to two years' experience working with people with developmental disabilities. Required to complete all MDS assigned trainings within first 6 months of hire and then annually.

WORKING CONDITIONS

The Residential and CPS Float Staff are considered Essential Employees. This classification means that the staff person will be required to work in program and the community as needed. Work schedules vary and include day, evening and overnight shifts throughout the week and weekend. Staff are accountable for all hours assigned, are punctual and regular in attendance, and attend appropriate training activities. Flexibility to accommodate individuals served is necessary. Valid driver's license, reliable transportation and 100,000 / 300,000 auto insurance is required. Mileage reimbursement is available for employment related travel. Position requires medication certification. Safety Care training may be required. Float will have the ability to lift up to 25 lbs. or more with assistance / lifts.

The Residential and CPS Service Float Staff must pass the following background checks; criminal record, motor vehicles record, Bureau of Adult and Aging Services (BAAS), Division of Child, Youth and Families (DCYF), Office of Inspector General (OIG).
