Job descriptionProviding comprehensive assessments, including physical and psychosocial evaluations Coordinating and developing care plans for geriatric patients Delivering primary care services and managing patient health outcomes Collaborating with physicians and interdisciplinary teams. Ensuring access to preventive care services Minimum of 1 year of clinical experience as a Nurse Practitioner, preferably in geriatrics Completion of an approved Nurse Practitioner Program New York State RN / NP licenses and DEA certification Proficiency in comprehensive patient assessments Experience in Care Plan Development 2+ years of experience in a similar role Master's degree in Nursing Applicable certifications in Primary Care or Geriatrics Fluency in Spanish, Cantonese, or Mandarin Previous experience in Managed Care environments
