Compensation : $130,000–$170,000 annually + Bonus + Comprehensive Benefits

About the Role

An established, patient-centered healthcare organization is seeking an experienced Nurse Practitioner (NP) to provide onsite care and clinical oversight for adults with chronic and complex conditions. You’ll play a vital role in improving outcomes for patients with conditions such as diabetes, hypertension, heart failure, and chronic respiratory disease.

Key Responsibilities

Deliver comprehensive care to adult patients with multiple chronic conditions.

Conduct thorough assessments, develop individualized care plans, and manage ongoing treatment.

Prescribe, titrate, and monitor medications in accordance with clinical guidelines.

Partner with interdisciplinary teams to coordinate patient care and optimize outcomes.

Participate in program development and quality improvement initiatives.

Qualifications

Required :

Minimum 5 years of clinical experience in Primary Care, Internal Medicine, or Gerontology.

Proven ability to manage chronic conditions (e.g., diabetes, hypertension, COPD, CHF).

Active Nurse Practitioner license in Virginia with full autonomous practice authority.

DEA license and comfort prescribing for chronic diseases associated with ESRD and other complex conditions.

Must reside locally within the Chesapeake / Norfolk, VA area.

Preferred :

Experience in a primary care or chronic disease management program.

Prior involvement in launching or expanding clinical programs or services.

Familiarity with results-driven, performance-based care models.

Compensation & Benefits
